To enroll your child in an Osborn District school, contact the office of the
school your child will attend. If you're not sure about which school to contact,
call the district office at (602) 707-2000.
When you register your child, you will need to bring the following:
For admission to kindergarten, children must be five (5) years of age prior
to September 1st of the current school year. Children registering
after 8:30 a.m. may be asked to start school the following morning. This avoids
disrupting classes, allows the teacher to prepare a place for your child, and
gives your child a chance to get settled before class begins.
Under state law, schools must have written proof of immunization before admitting a child to school. Generally, most types of records supplied by the health care provider are acceptable, as are records supplied by a previous school or child care center. For more information, contact your school, your doctor or Maricopa Health Services. Immunizations required include:
measles, mumps, rubella (MMR) (1 and 2)
diphtheria
tetanus
pertussis
poliomyelitis
Hepatitis B vaccination
Varicella (Chickenpox)
or a complete history of disease form
If your child's immunizations are not current your child will
not be allowed to begin school until they are brought
up-to-date. If your child should not be immunized because of a medical
condition, personal belief, or natural immunity (i.e., he/she has already had
the vaccine-preventable disease), you must file a Request for Exemption with the
school. Free or low-cost immunizations are available through clinics sponsored
by the Maricopa County Health Department Community Health Nursing Services.
Occasionally, it may be in a child's best interest to transfer to another
school outside the student's assigned neighborhood attendance area. The transfer
may be made if the accepting school has sufficient space, facilities, staff, and
other resources, and if the principals at both schools approve the transfer
request. Transfer Request Forms can be obtained from your local school office.
Because the district does not provide transportation outside established
attendance areas (except for special education and magnet school students),
parents or guardians are responsible for arranging transportation for
transferred students.
If you must withdraw your child from school, please visit the school office
at least 24 hours ahead of time to fill out a withdrawal form. All school
materials, such as library books or textbooks must be returned before withdrawal
is complete.
The Osborn School District Governing Board has established written policies
regarding the collection, storage, retrieval, use, and transfer of student
educational information collected and maintained pertinent to the education of
all students to ensure the confidentiality of the information and to guarantee
parents' and students' rights to privacy. These policies and procedures are in
compliance with:
Student education records are collected and maintained to help in the
instruction, guidance, and educational progress of the student; to provide
information to parents and staff; to provide a basis for the evaluation and
improvement of school programs; and for legitimate educational research. The
student records maintained by the district may include - but are not necessarily
limited to - identifying data, report cards and transcripts of academic work
completed; standardized achievement test scores, attendance data; reports of
psychological testing; health data; teacher or counselor observations; and
verified reports of serious or recurrent behavior patterns.
These records are maintained in the office of the Building Principal, School
Psychologist and Director of Special Services for Osborn under the supervision
of each Building Principal, School Psychologist or Director of Special Services,
and are available only to the teachers and staff working with the students. If
your son/daughter should transfer to another school, these records will be sent
to the new school upon their request. Otherwise, records are not released to
most agencies or persons without prior written consent of the parents.
You have the right to inspect and review any and all records related to your
child, including a listing of persons who have reviewed or have received copies
of the information. Parents wishing to review their children's records should
contact the building principal for an appointment. School personnel will be
available to explain the contents of the records to you. Copies of student
educational records will be make available to parents when it is not practicable
for you to inspect and review the records at the school. Charges for copies of
records will be actual costs of copying.
If you believe information in the record file is inaccurate or misleading,
you have the right to request that a correction be made and to add comments of
your own. If at any time an agreement between the principal and parent cannot be
reached, you may contact Dr. Wilma Basnett, Superintendent and request a
hearing.
You shall be informed when personally identifiable information collected,
maintained, or used is no longer needed to provide educational services to your
child. The information must be maintained for two years after the date your
child was last enrolled in this school district.
Copies of the District Student Education Record Confidentiality Policies and Procedures may be reviewed in the principal's office in each school. Federal law also permits a parent to file a complaint with the Family Educational Rights and Privacy Act Office in Washington, D.C., if you feel the school is violating public school records, policies and statutes.
All Osborn Schools have a mandatory uniform policy.
