Registration and Attendance Information

Osborn School District #8

Admission Procedures

To enroll your child in an Osborn District school, contact the office of the school your child will attend. If you're not sure about which school to contact, call the district office at (602) 707-2000.

When you register your child, you will need to bring the following:

  • Birth Certificate or affidavit explaining the inability to provide a copy of the birth certificate. A.R.S. §15-828
  • Proof of immunizations (your child will not be admitted unless the immunizations are up-to-date.)
  • Name/Address of previous school.
  • Proof of legal custody may also be required.

For admission to kindergarten, children must be five (5) years of age prior to September 1st of the current school year. Children registering after 8:30 a.m. may be asked to start school the following morning. This avoids disrupting classes, allows the teacher to prepare a place for your child, and gives your child a chance to get settled before class begins.

Immunization Requirements

Under state law, schools must have written proof of immunization before admitting a child to school. Generally, most types of records supplied by the health care provider are acceptable, as are records supplied by a previous school or child care center. For more information, contact your school, your doctor or Maricopa Health Services. Immunizations required include:

measles, mumps, rubella (MMR) (1 and 2)
diphtheria
tetanus
pertussis
poliomyelitis
Hepatitis B vaccination
Varicella (Chickenpox)
or a complete history of disease form

If your child's immunizations are not current your child will not be allowed to begin school until they are brought up-to-date. Free or low-cost immunizations are available through clinics sponsored by the Maricopa County Health Department Community Health Nursing Services.

Student Transfers

Occasionally, it may be in a child's best interest to transfer to another school outside the student's assigned neighborhood attendance area. The transfer may be made if the accepting school has sufficient space, facilities, staff, and other resources, and if the principals at both schools approve the transfer request. Transfer Request Forms can be obtained from your local school office. Because the district does not provide transportation outside established attendance areas (except for special education and magnet school students), parents or guardians are responsible for arranging transportation for transferred students.

Student Withdrawals

If you must withdraw your child from school, please visit the school office at least 24 hours ahead of time to fill out a withdrawal form. All school materials, such as library books or textbooks must be returned before withdrawal is complete.

Confidentiality of Records

The Osborn School District Governing Board has established written policies regarding the collection, storage, retrieval, use, and transfer of student educational information collected and maintained pertinent to the education of all students to ensure the confidentiality of the information and to guarantee parents' and students' rights to privacy. These policies and procedures are in compliance with:

  • The Family Education Rights and Privacy Act; Title 20, United State Code, Sections 1232g and 1232h; and the Federal Regulations (34 CFR, Part 99) issued to such Act;

  • The Education of all Handicapped Children Act; Title 20, United States Code, Sections 1412(2)(D) and 1417(c); and the Federal Regulations (34 CFT 300.560 - 300.574) issued pursuant to such act; and
  • Arizona Revised Statues, Title 15, Section 141.

Student education records are collected and maintained to help in the instruction, guidance, and educational progress of the student; to provide information to parents and staff; to provide a basis for the evaluation and improvement of school programs; and for legitimate educational research. The student records maintained by the district may include - but are not necessarily limited to - identifying data, report cards and transcripts of academic work completed; standardized achievement test scores, attendance data; reports of psychological testing; health data; teacher or counselor observations; and verified reports of serious or recurrent behavior patterns.

These records are maintained in the office of the Building Principal, School Psychologist and Director of Special Services for Osborn under the supervision of each Building Principal, School Psychologist or Director of Special Services, and are available only to the teachers and staff working with the students. If your son/daughter should transfer to another school, these records will be sent to the new school upon their request. Otherwise, records are not released to most agencies or persons without prior written consent of the parents.

You have the right to inspect and review any and all records related to your child, including a listing of persons who have reviewed or have received copies of the information. Parents wishing to review their children's records should contact the building principal for an appointment. School personnel will be available to explain the contents of the records to you. Copies of student educational records will be make available to parents when it is not practicable for you to inspect and review the records at the school. Charges for copies of records will be actual costs of copying.

If you believe information in the record file is inaccurate or misleading, you have the right to request that a correction be made and to add comments of your own. If at any time an agreement between the principal and parent cannot be reached, you may contact Dr. Wilma Basnett, Superintendent and request a hearing.

You shall be informed when personally identifiable information collected, maintained, or used is no longer needed to provide educational services to your child. The information must be maintained for two years after the date your child was last enrolled in this school district.

Copies of the District Student Education Record Confidentiality Policies and Procedures may be reviewed in the principal's office in each school. Federal law also permits a parent to file a complaint with the Family Educational Rights and Privacy Act Office in Washington, D.C., if you feel the school is violating public school records, policies and statutes.

Uniforms

All Osborn Schools have a mandatory uniform policy. The requirements for these schools are listed below.

CLARENDON SCHOOL
Tops: Solid white, gray, navy, maroon, or light blue polo shirts or button down shirts and blouses. No logos are allowed on shirts or sweatshirts. All tops must be buttons, a collar and short or long sleeves. No zippers on shirts.

Bottoms: All shorts, skirts, skorts, pants, or jumpers are to be navy blue or khaki. NO DENIM or logos. All shorts, skirts, and skorts must be no shorter than finger tip length when the arm is extended down at sides.

Belts: Belts are optional. If worn they must be navy blue, black or brown with no initials, studs or inappropriate symbols.

Shoes: For safety reasons, no platform shoes, thongs or slippers may be worn. Closed-heeled and closed-toed shoes are recommended. Shoes should be white, black, brown or navy blue. Shoe laces must match the shoe or be white or black. Heels of shoes may not exceed one inch in height. White, gray, khaki, or navy blue socks or tights may be worn.

Winter Wear: Solid-colored sweaters or sweatshirts in navy blue, light blue, tan, white, or gray may be worn over a uniform shirt, with the collar of the shirt showing. Plaid shirts or jackets are not acceptable attire. Jackets with no logos may be worn outside the classroom.

ENCANTO SCHOOL
For Boys the colors and types are as follows:

Pants and shorts: Blue or Khaki
Polo Style Shirt: Navy blue, light blue, white, or gray

For Girls the colors and types are as follows:

Pants, Shorts, Jumper or Skirt: Blue or Khaki
Polo Style Shirt: Navy Blue, light blue, white, or gray

LONGVIEW SCHOOL
Bottoms:  Navy or khaki pants, shorts, jumpers, or skorts.  No denim.

Tops: White or Navy polo-style tops are preferred if you are buying new uniforms. 

We will still be allowing the old color tops from both Longview and Montecito:  light blue, yellow, gray, maroon.

Winter Wear: Solid-colored sweaters, lightweight jackets, pullover sweatshirts (gray, navy, white, or maroon only) may be worn over a uniform shirt with the collar showing. Flannel or plaid shirts are not allowed. No logos will be allowed on shirts or sweatshirts. Jackets with logos may not be worn in the classroom. 

SOLANO SCHOOL
Tops: Solid white, red or navy blue. No logos will be allowed on shirts or sweatshirts. All tops must have a collar and long or short sleeves, and no zippers.

Bottoms: (pants, shorts, skirts, jumpers or skorts) All navy blue or khaki. No denim or logos.

Belts: Belts must be navy, black or brown with no initials on the belt or the buckle.

Shoes: For safety reasons, closed-toed and closed-heeled or athletic shoes are recommended. No platform shoes, sandals or slippers shall be worn. Shoelaces must be correctly laced, securely tied and standard size. White or navy socks or tights may be worn.

Winter Wear: Solid-colored sweaters or pullover sweatshirts (red, navy or white only) may be worn over a uniform shirt with the collar showing. Flannel or plaid shirts are not allowed. Jackets with logos may not be worn in the classroom.

OSBORN MIDDLE SCHOOL

Shirts
Shirts must be polo-style and have a full collar. No logos on shirts except school insignias. Shirts must have sleeves. Shirts may be long or short sleeved. Shirts must be white, navy blue, or maroon. Students may NOT wear all navy blue. Shirttails must be long enough to be tucked in when arms are raised above the head (as in a jumping jack). No exposed midsections or undergarments. Shirts may not be more than one size too large. Undershirts and/or turtlenecks may be worn underneath a polo-style shirt, may be black, white or gray, long or short sleeved, with no visible insignia.

Pants
Girls: Long pants, capri-style pants, shorts, skorts, or jumpers, no sweats or jeans.
Boys: Long pants, shorts, no sweats or jeans/denim
Both: Pants may be navy blue or khaki - industrial uniform-style pants (Dockers brand is an example of an appropriate pant style). All pants must remain at the waist and may not be more than one size too large. Pant legs must hang naturally, may not touch ground, and must be hemmed. Pants may not be rolled. No pants with cargo-style (puffy) pockets. Grunge/Alternative-style pants are not allowed. Shorts, skirts and skorts must be at least to the longest fingertip in length. Slits may not be shorter than the longest fingertip in length.

Belts - Girls & Boys:
Belts are optional - If one is worn, it must be blue, black, or brown. It may be made of fabric or leather. The belt must run through all loops. It cannot hang longer than the student's thumb and index finger spread apart - with the thumb starting on any part of the buckle. Belt buckles must be plain, with no initials, emblems, or symbols. If a student needs a belt and does not have one, he/she will be given a commercial zip tie which must be worn to keep the pants up. When shirts are tucked in, the belt and/or waistband must be visible.

Shoes - Girls & Boys:
Shoes must be worn and must match. Shoes will be predominantly white, black, or brown. Trim and logos on the shoes may be different colors as long as the shoe is predominantly white, black, or brown. Shoes must be closed-toe with a heel strap. Slippers, flip-flops (thongs), and slip-ons are not allowed.
