• Immunization Requirements

    Under state law, schools must have written proof of immunization before admitting a child to school. Generally, most types of records supplied by the health care provider are acceptable, as are records supplied by a previous school or child care center. For more information, contact your school, your doctor or Maricopa Health Services. Immunizations required include:

    Diptheria Tetanus Pertussis (DPT, DTap or DT) 4-5 doses required – one dose must be received after the age of 4
    Measles Mumps Rubella (MMR) - 2 doses required
    Poliomyelitis - 3-4 doses required
    Hepatitis B - 3 doses required
    Varicella (chicken pox) - 1 dose required
    Haemophilus influenzae type b (Hib) for a child under the age of 5
    Hepatitis A, for a child one (1) through five (5) years of age in a day care program in Maricopa County

    Students entering the 6th grade or above are required to have 2 additional immunizations that are listed below:

    Tdap - 1 dose required at the age of 11 or older
    M
    eningococcal Meningitis – 1 dose required at the age of 11 or older

    If your child's immunizations are not current your child will not be allowed to begin school until they are brought up-to-date. Free or low-cost immunizations are available through clinics sponsored by the Maricopa County Health Department Community Health Nursing Services.
